In a large tertiary centre an uncertain tracing goes to a cardiologist. In a district hospital, a clinic, a night shift, or much of the world outside major cities, it does not — the doctor holding it decides, and the decision may be whether to thrombolyse, whether to transfer, or whether it is safe to wait until morning.

The patterns that decide transfer
Disproportionate attention goes to the tracings that are routinely under-called — posterior infarction, the de Winter pattern, Wellens, the subtle high lateral occlusion, the paced patient with ischaemia, and the post-arrest ECG. These are precisely the tracings on which a peripheral physician’s decision differs from a cardiologist’s, and where the cost of a miss is highest.
Discipline over recognition
Every chapter enforces the same reading order. The commonest interpretive error in practice is not ignorance but sequence — the obvious abnormality is seen, the systematic read is abandoned, and the second abnormality is missed. A physician who has worked through this book reads in an order that keeps the striking finding in its proper place.
